One thing you can count on from your hysterectomy—your periods are gone forever. Menopause is defined as the last naturally occurring period that a woman has. Climacteric is the technical term for all the changes and symptoms that happen now. Usually all of this happens between the ages of forty-five and fifty-five. Most of us, though, think of the whole kit and caboodle as menopause and realize that the changes and symptoms are due to fluctuations and decline of our hormones. The question of whether to use hormone replacement therapy (HRT) in a woman involves the presence or absence of four factors: older age, osteoporosis risk factors, cardiovascular risk factors, and status of the uterus and ovaries. The effect of estrogen for the prevention of osteoporosis is thought to be dependent on a “window of opportunity” during which maximum effect can be achieved. The sooner hormone replacement therapy is started after the surgical or natural menopause, the greater the effect, with maximum effect achieved within 3 to 5 years afterward.

While HRT may help you recover after a hysterectomy, its long terms risks must be assessed. Initially the early observational studies conducted in the 1980s and 1990s highlighted the benefits of HRT but mounting evidence from further studies began to uncover a number of serious risks associated with taking HRT including higher risk of heart attack, stroke, thromboembolic disorders and cardiovascular events. Among women who took HRT, the risk of developing endometrial cancer, blood clots, stroke and coronary disease was statistically higher than those who didn’t.
