About Infertility and Peritonitis

Mirea is an intrauterine device, and although the Mirena is supposed to work as a contraceptive, it can sometimes fail and the woman can get pregnant. Women who do become pregnant while using Mirena are believed to have a high risk of ectopic pregnancy. An ectopic pregnancy is one that develops in the Fallopian tube instead of in the uterus. After a short time—at most a few weeks— the pregnancy outgrows the tube. Sometimes it is simply absorbed with no treatment being necessary but there is always the possibility of the tube’s rupturing with severe life-threatening hemorrhage.

Ectopic Pregnancy

An ectopic pregnancy is one in which the fertilized egg grows in a site outside of the uterus, most commonly in the Fallopian tube. An ectopic pregnancy cannot develop normally into a baby. As the pregnancy grows, it erodes into the blood vessels of the tube. Sometimes, the body will simply absorb this fertilized egg into itself without any medical intervention and the tube will heal spontaneously without surgery, but there is always the risk of bleeding from a large blood vessel. A ruptured ectopic pregnancy is a life-threatening emergency.

Treatment

Ruptured ectopic pregnancy has to be treated by an open surgery because the severe bleeding needs to be rapidly controlled. It used to be that all ectopics were treated this way, ruptured or not, but since laparoscopy has become a well-developed surgical technique, it is clear that unruptured pregnancies, or those with only minor bleeding, can be treated laparoscopically. This is now the most common method of surgical treatment. It should also be mentioned that early ectopic pregnancies can often be treated with methotrexate, a cancer drug, which, in small doses, causes the absorption of the pregnancy without the need for surgery. The medication is given by injection and the absorption of the ectopic is followed with more hormone tests and ultrasounds until everything returns to normal. A further alternative is to place some methotrexate into the tube close to the ectopic pregnancy at the time of laparoscopy; this causes the pregnancy to shrink and be absorbed without the need for cutting the tube. The type of treatment depends on the severity of the bleeding, the size of the ectopic pregnancy, whether the other tube looks normal and healthy, and the particular skills and views of the doctor.

Side Effects

Some women who have had Mirena implanted experienced serious and even debilitating or life-threatening side-effects. The device may migrate from its original position after being inserted, perforating the uterus or embedding itself in the uterus. In both instances, a doctor must locate and surgically remove the device. Mirena may also migrate outside the uterine cavity and cause adhesions or scarring that can lead to infertility. Depending on the damage caused, affected women may require a complete hysterectomy, and more serious complications could result in death.

Serious side effects that have been linked to Mirena include:

  • – Inflammation of the membrane that lines the abdominal cavity and internal organs (Peritonitis)
