No surgery is entirely without risk. You need to be aware not only of worst-case scenarios, but also of the various possibilities that may delay your healing, require further surgery or other treatment, and lengthen your recovery time. The risks of surgery are obviously greater for women as they get older and women with certain existing medical problems. If you are over sixty-five or have heart disease or chronic lung problems, you clearly may have more difficulty with your anesthetic than a fit thirty-five-year-old who runs ten miles a week.

Infections

Urinary tract infections and wound infections are the most common infections after hysterectomy. Wound infections can also cause a rise in temperature, accompanied by the development of redness at the incision site—on your belly after an abdominal hysterectomy or your perineum after vaginal surgery—and, sometimes, pus or a vaginal discharge as well. Often, simply removing the stitches cures the infection, but sometimes it is necessary to use antibiotics.

Organ Damage

The bowel, bladder, and ureters (the tubes leading to the kidneys) actually are to the uterus are situated close to the uterus. In most cases, it is very easy for your surgeon to move these organs out of harm’s way, but there are cases in which the pelvic organs are all stuck together by endometriosis, infection, or scar tissue from previous surgery. In these instances, the bladder and/or bowel may be damaged. If the bladder or ureter is damaged, and then repaired, a urinary catheter will be left in place for several days post-op to allow healing, and antibiotics will be prescribed. If the bowel has been damaged and repaired, you will be kept on a lowroughage diet for a while postoperatively, and you will also receive antibiotics.

Thrombosis and Embolism

Thrombosis is a condition in which blood stagnates in the blood vessels in the legs and forms clots. Bits of these clots may break off and travel through the blood vessels to the lungs, causing a blockage that interferes with the exchange of oxygen in the lung and can be fatal. This is called a pulmonary embolism. Keeping the legs immobile during any surgery, as well as certain blood changes caused by surgery itself, can predispose to thrombosis and embolism.
