Diathermy is used in several ways during surgery. It coagulates blood and seals off blood vessels, thus stopping bleeding. It can be used, instead of a scalpel or scissors, to cut through tissues cleanly, with very little bleeding at all. It is useful in destroying abnormal tissue, as we discussed in regard to endometriosis. Diathermy is also used to open ovarian cysts, cut out fibroids, or separate the uterus from its attachments during laparoscopic hysterectomy. All in all, it is a very useful tool.
Obviously, if the diathermy probe is not used with great care, the heat can damage the bowel or bladder or surrounding tissues, with the same risk of peritonitis and infection that we have seen following damage from other instruments. If diathermy burns occur, the same types of repair that we have already described will have to be done, usually as an open surgery.
The laser uses light energy to destroy well-defined areas of tissue in the same way as the diathermy. Lasers may be used for cutting tissue or stopping bleeding during the course of a surgery and they are used for treatment, for example, of endometriosis. The laser uses light at a very high wavelength to produce intense heat which vaporizes tissue. In skilled hands, the laser is a very precise and useful tool. Nevertheless, damage can occur and this may be to the bladder, bowel, or blood vessels. As with other forms of damage, immediate open surgery is usually necessary.
Another instrument, the harmonic scalpel, has recently made its appearance in laparoscopic surgery. This uses sound waves to cut and coagulate tissue and may be useful as an alternative to diathermy and laser. When damage occurs in the course of a laparoscopy, it is usually immediately obvious to the surgeon and she will proceed quickly to repair the damage. Sometimes, however, when small diathermy or laser burns happen, or the Veress needle causes a very minor puncture, the damage may not be noticeable during the surgery and only becomes obvious later. This is the reason for the close surveillance of your vital signs and general well-being postoperatively. All of these signs should be normal and you should have urinated at least once, without any difficulty, before you are discharged home. As well, should you develop a fever or increased pain in the days following your return home, you should immediately contact your doctor. It may be nothing, but better safe than sorry.
