On the front of its Business Day section, the New York Times (11/3, B1, Wilson, Subscription Publication) reported that on Nov. 3, British drugmaker GlaxoSmithKline announced that “it had agreed to pay $3 billion to settle United States government civil and criminal investigations into its sales practices for numerous drugs.” The Times added, “The settlement would be the largest yet in a wave of federal cases against pharmaceutical companies accused of illegal marketing, surpassing the previous record of $2.3 billion paid by Pfizer in 2009.”

“In a statement Thursday, Glaxo said it expects to finalize the terms of the settlement next year and put an end to the company’s ‘most significant’ US liabilities,” the Washington Post (11/3, El Boghdady) reported. “Those include a seven-year investigation into the marketing of 10 popular Glaxo drugs, a Justice Department probe into possible Medicaid reimbursement fraud and another into the development and marketing of the diabetes drug Avandia [rosiglitazone].”

The National Journal (11/3, Fox, Subscription Publication) pointed out, “The case is not completely settled, but the agreement brings the British drug company and the US government closer to resolving the issue.” Providing background on the story, the National Journal explained, “The US government has been trying to clamp down on drug companies’ illegal marketing practices, from promoting drugs for unapproved uses to sponsoring clinical trials designed to make drugs appear more effective than they actually are.”

“Negotiations over the terms will be completed next year, the London-based company said in a statement” yesterday, Bloomberg News (11/3, Serafino, Kitamura) reported. “The cost is covered by existing legal provisions and will be paid from the company’s cash resources, Glaxo said.”

Infant Heart Defects

Ingestion of Wellbutrin during pregnancy could cause infant heart defects, including hypoplastic left heart syndrome. Hypoplastic Left Heart Syndrome is a congenital heart defect. It involves a spectrum of severe heart defects with a common problem: the main pumping chamber of the heart – the one that sends blood with a new supply of oxygen to the organs of the body-has not developed adequately. In addition, part of the body’s main supply artery, the aorta, may be missing or underdeveloped.

Babies with this type of congenital heart defect can live for a time following birth as long as a connection between the normal right ventricle and the infant’s aorta remains open. That open normally closes hours to days following delivery. Medication can keep it open for weeks to months. Once the connection closes, the heart cannot deliver oxygenated blood to the tissues and the child will die.

The Norwood sequence of procedures is a serious of operations that make it possible for the healthy right ventricle which normally only pumps blood through the lungs and back to the heart to do the work that usually two ventricles working together provide. The first operation can take place in the first days of life. If successful, it will sustain the baby’s life for months and allow time for important growth and development. The operations do not afford complete correction of the heart. It may provide sufficient heart function to support average or better daily activity for many years but at some point, the single pumping chamber may fail to provide everything the body needs. The operation also carries considerable risk of morbidity and mortality.

An alternative to surgery is heart transplantation – the removal of the defective heart with replacement by one with normal structure and function. The baby must wait for a donor heart to become available. A large proportion of babies awaiting for a new heart die of complications of their disease and treatment while waiting for a new heart. Some babies succumb to infection related to the intravenous catheters need to provide drugs that sustain circulation before surgery. Rejection and/or medication to fight it can damage the new heart and make the child more vulnerable to death from infection.

About Wellbutrin

Wellbutrin is an antidepressant drug prescribed to treat major depressive disorder, also called major depression and clinical depression, and seasonal affective disorder, a type of depression involving adverse side effects that occur each year during the same season. First approved by the FDA in 1985, bupropion hydrochloride was available for several years only under the brand name Wellbutrin. In 1996, the FDA approved a sustained-release tablet of this drug called Wellbutrin SR and, in 2003, approved an extended-release tablet known as Wellbutrin XL.

Wellbutrin carries a Pregnancy Category C classification, meaning that animal reproduction studies have shown an adverse effect on the fetus but that no adequate or well-controlled studies in humans have been performed.

Wellbutrin has been linked to congenital heart defects. Congenital heart defects affect approximately 40,000 babies each year, and one of the primary known causes of such birth defects is the ingestion of teratogenic drugs during pregnancy. A teratogen is a drug or other substance capable of interfering with the development of a fetus that can ultimately result in serious birth defects.

Infant Heart Defects

Ingestion of Wellbutrin during pregnancy could cause infant heart defects.  

A fetus acquires its blood oxygen and disposes of carbon dioxide through its umbilical cord. So the circulation of blood through the lings is unnecessary. Until birth the function of the heart is to pump blood from the inferior vena cava which carried oxygenated blood from the umbilical cord to the aorta which distributes the blood through the body. Accordingly the lungs are switched out of the circuit of blood by a valve that directs incoming blood from the right atrium to the left atrium. At birth this “short circuit” normally ceases to function and the interatrial valve heals up as the lungs assume their role in oxygenating blood and eliminating carbon dioxide.

In some cases however the interatrial valve fails to heal completely, so that a proportion of the blood that arrives at the right atrium passes to the other side of the heart rather than following the normal course through the right ventricle and thence to the lings. This is one of several abnormal heart conditions that arise because the connections in the fetus fail to rearrange completely for the normal function of the heart outside the womb. Collectively these conditions are described as congenital meaning with birth and they have various degrees of impact on the ability to lead a healthy active life.

In another congenital heart defect, a passage that connects the pulmonary artery to the aorta fails to close at birth. The proportion of blood that reaches the lungs is so small that the baby becomes filly deoxygenated. This condition is called blue baby syndrome because of the abnormal coloration caused by dark blue deoxygenated blood. This condition is life threatening but can often be remedied by emergency surgery.

Such a condition may cause surgery, lifetime defects or death.

Every year, about 30,000 to 40,000 babies are born with defective hearts. An inborn or congenital heart defect means that the heart, or a blood vessel near the heart, failed to develop normally during the prenatal period of growth. Inborn heart defects are sometimes mild and sometimes serious. If the trouble is mild, surgery may never be necessary, and the person may lead a normal life. More serious defects can often be helped by surgery, and each year research paves the way for more defective hearts to be helped by surgery. Even when this is not possible, good medical care is beneficial.

When surgery is used, it may correct the defect completely or partially so that circulation becomes normal or at least nearly normal. Youngsters with congenital heart defects need close medical supervision during their growing years so as to avoid any infections which might cause difficulties. Standard immunizations are a “must.”

Congenital heart disease can be fatal to an infant. Congenital heart defects include: 

• – Pulmonary valve atresia (also known as hypoplastic right-heart syndrome), in which blood flow to the baby’s lungs is blocked
• – Tricuspid atresia, in which there is no opening to allow blood to pass through the right-hand side of the heart
• – Pallet’s (pronounced fallows) tetralogy, in which there is a hole in the baby’s heart and also a blockage preventing blood from reaching the baby’s lungs

These and many other congenital heart defects can put a baby’s life at risk.  In some hospitals, some of these conditions can now be diagnosed by detailed ultrasound scan while the baby is still in the womb. But often a severe heart defect does not become apparent until several days after the baby is born, since it takes some time for the circulatory system to finalize the changes that occur at delivery. Even in those conditions that can sometimes be successfully remedied by surgery, all operations carry some risk, especially in such tiny babies, and some babies do not survive.
