Hypoplastic Left Heart Syndrome is a congenital condition of the heart in which the left ventricle is malformed and very small. Hypoplastic means underdeveloped. The left ventricle of the heart and the beginning of the main artery to the body (the ascending aorta) do not develop normally and are very small. As a result, oxygenated blood returning from the lungs cannot pass through the heart to the body except through the ductus arteriosus. The ductus arteriosus is an artery which goes between the main artery to the lungs (the pulmonary artery) and the arch of the aorta. When a baby is in the womb, this is the normal way for the blood to go and so hypoplastic left-heart syndrome does not cause major problems before birth. However, once the baby is born, the ductus may close at any time, usually within one or two days. There is then no way in which oxygenated blood can get to the body. When the ductus starts to close, a baby with this condition will deteriorate very rapidly and will die unless treated.

In this condition the pulmonary artery, which carries deoxygenated blood to the lungs, and the aorta, which carries oxygenated blood round the body, are crossed over and come out of the wrong sides of the heart. This creates two completely separate circulatory systems in which the blood to and from the lungs all flows into the left-hand side of the heart. No oxygenated blood can flow to the organs in the rest of the body. While the baby is in the womb this does not matter because the baby does not depend on oxygen from his or her lungs, and the foramen ovale and the ductus arteriosus allow blood to circulate around the whole circulatory system. However, once the baby is delivered and begins to breathe on its own, the foramen ovale and the ductus arteriosus close. Unless there are other ways for the blood to get through, no oxygenated blood can flow around the baby’s body. In many cases, an emergency operation can now be successfully carried out to allow the baby’s blood to flow around the body, but some babies with this condition still die.

    A cleft is a separation of parts of the mouth usually joined together during the early weeks of fetal development. A cleft lip is separation of the upper lip and often the upper dental ridge. A cleft palate is a separation of the hard or soft palate and occurs with or without a cleft lip. Because of the lack of closure between the oral and nasal cavities, newborns have difficulty maintaining sufficient negative pressure to express liquid from a nipple. The infant’s tongue does not have an upper surface to express milk from the breast or bottle.

    Cleft lips and palates are closed through surgery. Surgeons can repair cleft lips in the first few months of life. Repair of a cleft palate is more extensive, and the surgeon generally waits until the infant reaches a certain weight, usually by 12 months of age. Generally, surgeons perform multiple procedures to correct the abnormalities. These surgeries include bone grafting, orthodontic repair, and placement of ear tubes.

    Feeding can be a major challenge because of the way the infant’s mouth is affected. In order to feed, the baby must grip with the mouth and close off the nasal cavity to create a chamber of reduced pressure into which milk is drawn. When a suitable amount of milk has been taken into the mouth, a swallow is triggered – and all this must be achieved while continuing to breathe. Feeding can be stressful and exhausting, and takes much longer than for unaffected infants. In some cases an infant may have to be fed via a syringe, and sometimes a gastric tube is inserted surgically to enable an infant or small child to feed sufficiently. If a newborn is not to remain in hospital, the parents may have to learn to use special equipment and nutritional supplements or their baby will fail to thrive.

      Coarctation of the Aorta

      Ingestion of Wellbutrin during pregnancy could cause coarctation of the aorta in your child.  Coarctation of the aorta refers to the narrowing of the main artery from the heart. This is a congenital abnormality which has to be corrected surgically.

      Coarctation of the aorta is associated with congenital anomalies of the mitral valve. The leaflets, chordae, papillary muscles, and the left ventricular wall are commonly involved. Clefts, perforations, and myxomatous changes may be present in the mitral valve leaflets, and leaflet tissue may be deficient. In addition, abnormally long or short chordae with poorly developed interchordal spaces may be seen. Furthermore, abnormalities of papillary muscle and endocardial fibroelastosis of the left ventricular wall are also sometimes present. These abnormalities frequently produce a murmur of mitral regurgitation.

      Coarctation of the aorta often occurs as a discrete stenosis at the level of the ligamentum arteriosum, but it may be associated with proximal aortic hypoplasia. Usually associated with a bicuspid aortic valve, coarctation of the aorta may also be associated with aneurysm of the circle of Willis, VSD, and Turner’s syndrome.

      Coarctation of the aorta is a congenital condition in which the heart’s main artery narrows. The aorta carries blood from the heart to the vessels that supply the body with blood and nutrients. It is hard for blood to pass through the artery when part of the aorta is narrowed. So the baby’s heart has to work harder to allow the blood to pass through the artery. This makes the baby breathless and too tired.

      While in some newborns the symptoms will appear within the first few days of birth, in some cases, especially milder ones, the symptoms will appear only at adolescence. The symptoms of coarctation of the aorta includes shortness of breath, chest pain, nose bleeding, hypertension, poor growth, fainting or dizziness, pounding headache, cold feet or legs, leg cramps and inability to exercise.

        Neural Tube Defect

        Ingestion of Wellbutrin during pregnancy could cause neural tube defect in your child.  Neural tube defect is the medical term for malformations of the spinal cord and the brain. The brain is a soft organ surrounded by membranes called meninges and enclosed within the skull. The spinal cord, also surrounded by meninges, leads out of the brain and down the spine. Both the brain and the spinal cord are bathed in cerebro-spinal fluid produced in the brain. The brain and the spinal cord known as the central nervous system or CNS control all the body’s activities.

        A developing baby’s spinal cord and brain begin to form between fifteen and twenty-five days gestation and are called the neural tube. A groove forms along a flat sheet of cells which will eventually become the baby’s back. The two sides of the groove then meet across the top and join to form the brain, the spinal cord and the spinal column, which protects the vulnerable spinal cord. If the two sides of the spinal column do not meet and join properly, the spinal cord may be left incompletely developed, exposed and vulnerable. The brain may also not form properly. This causes what is called a neural tube defect.

        Anencephaly is the most severe neural tube defect. In an anencephalic baby the upper part of the brain (cerebrum) and most of the lower part (cerebellum) do not develop during the first few weeks of pregnancy. Anencephaly means ‘without a brain’. The bones of the skull may also not form properly and the top of the baby’s head may be misshapen. Anencephaly is usually diagnosed in the womb by AFP tests or ultrasound. Some babies with anencephaly miscarry. A few are stillborn, and those who are born alive die within a few hours or days of birth.

        If parents have had one baby with a neural tube defect, there is thought to be a one in 25 chance in each future pregnancy that the baby will have either spina bifida or anencephaly. If parents have had two babies with neural tube defects, the risk for future pregnancies increases to one in 7.

          Craniostenoisis

          Ingestion of Wellbutrin during pregnancy could cause craniostenosis.  Craniostenosis or craniosynostosis is the premature closure of the flat bones of the skull while the brain is still growing in size. This results in pressure on the brain and if untreated will cause mental handicap. It may be associated with an unusual facial appearance depending on the bones affected but if the majority of the bones fuse together the result is called acrocephaly or oxycephaly. An operation is required to separate the bones and to reduce the pressure on the brain.

          The symptoms of craniosynostosis include disappearance of the fontanelle early, unusual head shape, absence of the normal feeling of a “soft spot” (fontanelle) on the newborn’s skull, a raised hard ridge along the affected sutures and slow or no increase in the head size over time as the baby grows.

          Craniostenosis usually implies that the sutures of the human skull, which are normally about 80 percent closed by two to three years of age, begin instead to close at nine to ten months of age. At this developmental time (under normal conditions) the growing brain is literally pushing the overlying, and still very mobile, skull to ever greater expansion. If the sutures close prematurely, the rapidly enlarging brain begins to fill a closed cranial vault with resulting secondary pressure and atrophy of the underlying brain. Neurosurgical intervention involves crushing the suture lines, placing strips of platinum or polyethylene in the suture lines, and allowing this material to remain in place until the child is almost three years of age.
